Information about Animal Registration Forms for 4-H Livestock Projects

Animal Registration Forms

The following forms should be used by every Junior Fair exhibitor wishing to take livestock projects to the fair.  Most forms are due by the second Wednesday in May at the Athens County Extension Office.  However, forms will also be taken at the Fairboard Secretary's Office that same day at the tag-in that takes place on the Fairgrounds.  Included next to the form title is a detail of when each form is due.

Breeding Beef Registration Form - Due the Second Wednesday in May

Breeding Gilt Registration Form - Due the Second Wednesday in May

Breeding Lamb Registration Form - Due the Second Wednesday in May

Breeding Poultry Registration Form - Due the Second Wednesday in May

Breeding Rabbit Registration Form - Due the Second Wednesday in May

Dairy Cattle Registration Form - Due the Second Wednesday in May

Dog Certificate of Vaccination Form - Due June 1
